The ASQ:SE system includes
8 questionnaires and corresponding scoring sheets (on CD-ROM and paper now included in both formats in every ASQ:SE box!). Parents complete the short, simple questionnaires at eight designated intervals: 6, 12, 18, 24, 30, 36, 48, and 60 months. Set up like the ASQ-3, these questionnaires prompt parents to answer 22 36 questions with most of the time, sometimes, and rarely or never. Each questionnaire takes just 10 15 minutes for parents to complete and addresses seven crucial behavioral areas: self-regulation, compliance, communication, adaptive functioning, autonomy, affect, and interaction with people.
For every questionnaire, there s an easy-to-use form that allows professionals to score the test in 1-3 minutes and determine the child s progress in that social-emotional area. The reproducible scoring sheets all include referral considerations that help determine if the child needs further evaluation.
